To save time, please download, print, and complete the intake forms below and bring them with you to your first session. If you aren’t able to do this for whatever reason, don’t worry—I keep printed forms at my office.
If you have questions about any of my policies or the questions in the forms, I will be happy to answer those questions for you when we review the forms at the beginning of our time together. If the question is urgent, feel free to contact me before then.
Missed, rescheduled, & Cancelled appointment Policy
Fees for appointments cancelled or rescheduled less than 48 hours in advance of the scheduled appointment time are due in full. This allows me sufficient time to attempt to fill the session. For example, in order to avoid the fee, an appointment for Wednesday at 4:00pm must be cancelled or rescheduled before 4:00pm the prior Monday. Exceptions may apply for illness or emergencies, to be determined at the discretion of Daniel Akins, LMT.
You should receive an auto-generated confirmation email whenever an appointment is scheduled. From within this email, you can reschedule or cancel your appointment. Please search for this email first and only email, text, or call to cancel or reschedule if you are unable to do so from within the confirmation email. After you have cancelled or rescheduled, please send an email to email@example.com or a text to (503) 410-3776 to let me know what’s going on.
I will wait up to 30 minutes past the scheduled time for you to show up to an appointment; after that, if I have not heard from you then I will consider it a no-show cancellation and may leave the office. Please check in by calling or texting if you expect to be more than 30 minutes late for an appointment.
Should you not show up for an appointment, or cancel or reschedule less than 48 hours in advance, then I will first send you an email to check in. If I do not hear back from you within 48 hours from the time that email is sent, then your card on file will be charged for the full rate of the missed session and I will send you an email notifying you that your card has been charged.
